$80 a Month Per Person

Unlimited office visits
Diagnosis and treatment
Annual physicals and preventive screenings
Chronic disease management
Referrals to specialist

Health education and counseling
Urgent care for non-emergencies
Basic procedures (e.g., stitches, abscess, etc)
Access to your medical provider via phone, email, or text
Labs billed at our cost (use insurance for labs if your plan covers them) and discounted imaging
$68 for each additional family member after the first two
Add weight management ($169 total) or hormone care ($139 total) for $40 less
